APPLY FOR, REPLACE, OR RENEW YOUR LIBRARY CARD

APPLY FOR A LIBRARY CARD

You may apply for a library card in person at the circulation desk of either of our buildings during regular hours. You will need some form of ID that shows your CURRENT address such as a driver’s license, or a piece of mail that was sent to your current address, in order to receive your library card.

To save time, you may apply for a card online at: Online Registration . You will still need to come into the library with ID as outlined above to receive your library card, as the temporary number you will be given will NOT give you access to our databases or allow you to place requests for items.

Children under the age of 18 must be accompanied by a parent or legal guardian and have a parent’s/guardian’s signature when obtaining a card.

A library card obtained at the Sullivan Free Library may be used at any of the MidYork libraries in Oneida, Herkimer, or Madison Counties. If you already have a card with any of the MidYork libraries, you may use it in Chittenango and Bridgeport. You will not need another card.

REPLACE A LOST CARD

If you have lost your library card, you can get a replacement card at the circulation desk at either of our buildings. The first replacement card is free. There may be a 50¢ fee for subsequent card.

RENEW YOUR CARD PRIVILEGES

Your library card is good for three years. At the end of that time you will have to renew your privilege at our circ desks during regular business hours.
